Camper Next Steps

As Soon As Possible:

• Physical Form
  Download the form below, then sign, scan and email (or send by U.S. Postal Service) the completed form to the address for the camp you're applying for (provided at the top of that camp's description page). Note: The physical must be within 12 months of the start of camp.
  
  Print this PDF Physical Exam Form

After you receive an acceptance email from the camp director:

• Pay Online
  If you have not already paid, an online payment (via credit card) can be made by going to your COGWA Portal and finding the balance due for the camp you will attend. If you do not wish to pay online, you can send your check in the mail.
   
• Transportation Information
  Complete the online Transportation Information form found in the Portal. Use this guide to easily find the transportation form within the application.

Staff Next Steps

As Soon As Possible:

• Physical Form
  Print this PDF Physical Exam Form. Then sign, scan and email (or send by U.S. Postal Service) the completed form to the address for the camp you're applying for (provided at the top of that camp's description page). Note: The physical must be within 12 months of the start of camp.

• Background Check
  Submit your information for the mandatory background check if you are 18 or older and will serve as a camp staff member. This step only needs to be done one time each calendar year (regardless of how many camps you apply for in that year). Please keep track of your completion of the process so we don't have to pay another fee for an unneeded additional background check. If you have not already done so for this year, please begin the background check now. We will not be able to consider your staff application without it.
  
  Submit Background Check Information
